Supporting documentation

When you register with us and apply for support, you'll need to provide one or more of the following types of documentation. If you don't have these, please contact us for advice.

Disabilities and long-term health conditions

Disabilities and long-term health conditions include:

  • mental health conditions, such as depression and anxiety
  • visual impairments
  • autism spectrum conditions
  • long-term medical conditions, such as epilepsy or Chronic Fatigue Syndrome

If you already have evidence of your disability, please upload it when you register online or bring it when you visit us.

If you don't have evidence, please download and take the Medical Evidence Request form below to your GP or another medical professional for them to complete. Please note all evidence needs to be in English and on headed paper.

Dyslexia, dyspraxia and dyscalculia

You'll need to provide a full diagnostic report that was completed by a qualified assessor (a Charted Educational Psychologist or Specialist Teacher with an Assessment Practising Certificate).

AD(HD) and Autism 

You'll need to provide a signed letter confirming a diagnosis or referral from a suitably qualified professional (eg a psychiatrist or occupational therapist). 

Please note that the evidence needs to be in English and on headed paper.
